At a vestry [?] in the Union Church of St. Anne's in the town of Ballinasloe by due notice given, for the parishes of Kilcloony and Creagh. Met on Monday the 11 day of October 1813. Vicar, Curate, Church Warden and Parishoners thus in Vestry assembled:
Resolved that a sum not exceeding three pence per acre be levied from the parishoners of Creagh and Kilcloony as a fund for the establishment of a charitable dispensary for the purpose of affording medicine and medical advice to the lower orders of the people resident within the parishes. And that the Church Wardens do make out an applotment accordingly.
That the town of Ballinasloe do furnish the sum of £30 0s 0d each year to be levied by applotment from each house accordingly to a proportion or rate to be [?] by a committee consisting of 5 inhabitants in aid of the funds for said dispensary.
That the following persons do compose the said committee: The Priest, The Rev. Mr. Galbraith, Dr. Thomas, Mr. J. Usher Jun. & Mr. Cruise, any three therof to form a quorum. That a regulating committee be elected once in each year at the Easter Vestry to preside over the [?] of the said establishment which committee is to consist of 2 ladies & 2 gentlemen resident of said parishes to which shall be added (ex officio) the clergymen of both religous persuasions. And that the committee for the current year shall be Mrs. McDonough & Mr. Kelly for the parish of Creagh and Mrs. Maher and Mr. Dillon of Cahir for Kilcloony together with the clergy of both religious persuasions as aforesaid.
That a charity sermon be preached once in each year at Church or Chapel in or about the great fair in aid of the said dispensary.
